A GP Management Plan helps coordinate your care and may allow access to Medicare-supported allied health services where appropriate. Regular reviews ensure your treatment remains effective and aligned with your condition.

02 Am I eligible for a CDM plan?
You may be eligible if you have a chronic condition that has been present (or is expected to be present) for six months or longer.
03 Does a CDM plan cover allied health services?
Yes. Medicare provides rebates for eligible CDMP consultations and for up to five allied health services per calendar year when arranged under a valid plan.
