A Chronic Disease Management (CDM) Plan is a structured plan created by your GP to help manage long-term health conditions.
02 Am I eligible for a CDM plan?
You may be eligible if you have a chronic condition that has been present (or is expected to be present) for six months or longer.
03 Does a CDM plan cover allied health services?
Yes. Medicare provides rebates for eligible CDMP consultations and for up to five allied health services per calendar year when arranged under a valid plan.
04 How often do I need to review my plan?
Regular reviews are recommended to ensure your treatment remains effective and up to date.
